The ProblemPatients discharged from rehab had no supervision during the 2-3 days between hospital visits. Therapists had no visibility into what patients were doing at home, all rehab records were on paper, and the OPD queue was a physical token system causing daily congestion for thousands of patients.
The SolutionBuilt a Flutter mobile app where therapists assign home-based exercise programs, monitor patient activity remotely, and track progress digitally. Digitised all in-house rehab forms and workflows. For OPD, built a token and queue management system integrated directly with NICVD's internal hospital servers to sync patient and clinical records.
The OutcomeTherapists can now track patients continuously, not just during hospital visits. All physical forms are eliminated. OPD queue management is digital, reducing congestion and wait time for 2,000+ daily patients. The app is live on the Play Store and actively used across both departments.
NICVD's rehab department was running entirely on paper. Therapists tracked patient progress in physical folders, home-based recovery had no visibility whatsoever, and the OPD with 2,000+ daily patients managed its queue with paper tokens. I built the full system from scratch: a Flutter mobile app, a Node.js backend, PostgreSQL on Supabase, Docker-based environments, and a GitHub Actions CI/CD pipeline straight to the Play Store. The OPD module is integrated directly with NICVD's internal IT servers, syncing patient and clinical staff records with their existing infrastructure.
